New Membership Application
We offer two options for your membership application. You can either download, print, and mail in the application below, or follow the online option link. The following types of practitioners may apply:

ACTIVE I - First Year of practice in the local area: $273.75 (25% discount to Annual Dues)
ACTIVE - Established Physician (MD, DO & DPM): $365.00 Annual Dues
ACTIVE-LIMITED/SEMI-RETIRED - Practicing not more than 20 hours per week: $165.00 Annual Dues
FULLY RETIRED - Provider who is fully retired and has been a member for more than 30 years: $65.00 Annual Dues
Provider who is fully retired and has been a member for less than 30 years: Annual Dues is 1/3 of full dues, no less than $65.00
PHYSICIAN ASSISTANT/ADVANCED PRACTICE NURSE - $150.00 (59% discount to Annual Dues)

Membership Directory 2016-2017 Resource Guide
We publish an annual membership directory containing photos of each member along with information about their practice location. The membership directory also has listings of useful medical related resources.

Published annually, 700 membership directories are distributed to health care providers in Marion and Polk Counties, as well as to area pharmacists, medical office staff, hospital staff, administrators, provider spouses, community groups and individuals. Your message reaches the medical community continuously throughout the year. This valuable resource tool contains photos, office addresses, and telephone and fax numbers for each of the Society's 500+ members. The membership directory includes pharmacy listings, medical resource listings and vendor advertisements. Each Medical Society member receives one membership directory free of charge. Members may purchase additional membership directories for $20 each. Members of the public may purchase the membership directory for $20. MEdNet - The Medical Education Network
Here's your chance to get involved with children of all ages to share you knowledge and expertise and give back to your community! It's fun and rewarding.

As a clinician, you have an opportunity to share your love of medicine and science with children of all ages through the Society's MEdNet - The Medical Education Network© Program. If you are a Marion-Polk County Medical Society member and are interested in participating, please give us a call at (503) 362-9669. We'll add your name to our growing list of doctors who are willing to make presentations about the art and science of medicine, health and wellness topics or mentor a student interested in a career in medicine.

Mini-Internship Program
Once a year you have the chance to let invited members of the public see exactly what it is you do. Learn more about this experience and how you can participate.

Since 1986, over 40 clinicians have volunteered to "job shadow" selected members of the public. This program gives clinicians the opportunity to learn what concerns the public may have about health care issues, and it gives the interns a chance to see what the "day-in-the-life" of a doctor is all about. We encourage you to participate in this program.
